Developing a new and more successful treatment plan involves considering past treatment approaches and their efficacy. When creating a treatment strategy that leads to positive patient and healthcare outcomes, it is imperative to take into account data from previous treatment methods and their success rates.
In practice, patients experiencing adverse reactions to medications are common occurrences. Knowing whether a patient has had negative reactions to a previously administered medication is essential. In my experience as a psychiatric nurse, I have encountered patients who had adverse reactions to psychiatric drugs, which complicated their care and created additional challenges. To address such situations effectively, access to information about previous hospitalizations, treatment outcomes, adverse drug reactions, physician notes, and a clear diagnosis is essential (Nagle et al., 2017).
Understanding a patient’s past experiences with medications or specific environments is invaluable when counseling patients, especially in psychiatric settings. Identifying any previous manic, aggressive, or self-destructive behaviors in response to medication is crucial to ensure patient safety. Flagging the patient’s record with this information can prevent future incidents. Allergies should also be identified and documented in the patient’s chart, enabling nurses to prevent medication errors and potentially life-threatening situations. I have encountered such situations in the past, and prompt modifications to the treatment plan were necessary to ensure patient well-being.
While data from patient records is valuable, the involvement of clinicians in creating and implementing care plans is essential. Healthcare informaticists play a crucial role in developing robust, accessible tools for healthcare teams. However, direct contact with the patient by the care team often reveals subtle changes and nuances that data alone may not capture. Therefore, before sharing data, I prefer to discuss the patient’s habits and recent changes with the prior care team. Clinical nursing judgment involves cognitive, psychomotor, and emotional processes, while clinical reasoning encompasses the metacognitive and cognitive process of interpreting relevant knowledge for a specific patient or clinical situation (Nagle et al., 2017). These processes contribute to knowledge generation and the development of normative orders in nursing and healthcare.
