1. Provide an introduction to the case, why it has been selected, and any key issues that will be presented. (10 pts.)
2. Present the case comprehensively, being sure to include: (20 pts.)
3. Physical, psychological, social, developmental/family needs
4. HPI
5. PMH, FH, SH
6. H&P
7. Differential diagnoses
8. Initial management plan including any diagnostic testing or referrals to specialty providers
9. Address the treatment goals and identify projected outcomes and how they will be measured.  (10 pts.)
10. Review and analyze the current research pertinent to the case and relate to the management plan. (20 pts.)
11. Provide the confirmed diagnosis, any modifications made in the management plan including recommendations for further evaluation. (10 pts.)
12. Describe cost implications for the management plan. (10 pts.)
13. Summarize the significance of the case to the APN role. (10 pts.)
14. Overall presentation quality and subsequent seminar discussion including presentation of 5 relevant certification examination questions. (10 pts.)

Patient and diagnosis in any of the SOAP notes cannot be the same as Case Study Presentation.

HPI: recently exposed to cousins’ dog and started having symptoms. Symptoms worsed and albuterol has not helped.

last asthma exacerbation was 3 years ago when at summer camp during arts and crafts. Albuterol helped. Never needed steroids before.

Working Diagnosis: Acute asthma exacerbation

Start flovent BID and prednisone 20mg BID for 5 days and continue to use albuterol HFA PRN wheezing/SOB
