The repercussions of bias against TGD individuals in healthcare settings are especially concerning, as they can lead to profound health disparities and hinder access to essential care, as highlighted by Sherman et al. in their 2021 research. By incorporating Tanner’s story into these educational modules, students gain invaluable insights into the complexities of providing care to transgender patients, an opportunity that may be limited in traditional clinical settings.

Moreover, Tanner’s presence extends to the Advanced Diagnostics DCE, where students collaborate with Preceptor Diana to unravel the medical complexities of his clinic visit. In crafting Tanner’s portrayal for the Community Health DCE, Shadow Health collaborated closely with Trans Equity Consulting, an organization renowned for its dedication to advocating for transgender rights and equality. Together, they meticulously crafted an authentic representation of Tanner and his journey, ensuring that his story resonates truthfully with learners.

Similar to another character, Lupe, Tanner’s simulation includes a crucial component centered on building rapport with transgender clients. Consultants emphasize the paramount importance of establishing trust and understanding in healthcare interactions with transgender individuals, given the prevalent history of mistreatment and discrimination within the healthcare system. Students are encouraged to inquire about Tanner’s preferred pronouns and name, as well as his comfort level with specific terminology regarding his anatomy, exemplified by his preference for the term “chest” over “breast.”

Furthermore, Shadow Health conducted groundbreaking research, documented in Nurse Educator, which explored the impact of virtual patient simulations on nursing students’ attitudes and beliefs regarding transgender patients. The findings underscored the effectiveness of experiential learning in fostering cultural competence and sensitivity among nursing students, particularly in the context of caring for transgender individuals.

Tanner takes charge of making sure he stays healthy and happy. In some lessons about advanced medicine and community health, Tanner’s story is included twice. These lessons show how tough it can be for people like Tanner who are transgender to face discrimination every day, especially when they need medical care or are looking for a job. This discrimination can cause big differences in their health and make it hard for them to get the right care.

In another lesson about advanced medical tests, students work together with a teacher named Diana to figure out what might be wrong with Tanner when he visits a clinic. By having Tanner in three different lessons, nursing students get more chances to learn about treating transgender patients, which might not happen in regular lessons.

For the part of Tanner’s story that’s about community health, the company that makes the lessons worked with a group called Trans Equity Consulting. They helped make sure that Tanner’s story is real and true to life.

Apart from asking about Tanner’s preferred pronouns and name, students also learn what words Tanner likes to use to talk about his body (he prefers “chest” instead of “breast”) and talk about what Tanner is comfortable with before starting a physical exam.

Besides all this, the company that made the lessons did a study to see how it affected nursing students when they learned about caring for a transgender patient through a computer simulation. The study, which was published in a nursing journal, showed that students’ attitudes and beliefs about transgender people improved when they learned through these lessons. He also appears in the Advanced Diagnostics DCE where students collaborate with Preceptor Diana to determine a working medical diagnosis from his visit to the clinic. By featuring Tanner in three different simulation experiences, students across nursing education have a higher chance of interacting with a transgender patient, which they may not have the opportunity to in traditional clinicals. 

For Tanner’s appearance in the Community Health DCE, Shadow Health worked with Trans Equity Consulting, the same consulting agency referenced for Lupe’s case. With their guidance, Shadow Health’s development teams crafted a whole, authentic representation of Tanner and his experiences in Shadowville.

Similar to Lupe, Tanner’s simulation also includes a Rapport Building activity. According to consultant input, establishing rapport with a trans client is extremely important during a visit. This population has often had negative experiences in the healthcare system—research shows that 25% of trans patients report delaying healthcare-seeking out of fear of mistreatment and discrimination. In addition to asking about pronouns and his preferred name, students will also determine what terms Tanner prefers for referencing his anatomy (he prefers “chest” instead of “breast”); and discussing Tanner’s preferences for physical examination prior to beginning the exam.

Additionally, Shadow Health recently conducted research, published in Nurse Educator, to determine the effect of a virtual patient simulation scenario of caring for a transgender adult on nursing students’ attitudes and beliefs about transgender people. Study findings support experiential learning in nursing as an effective tool in teaching cultural competence and sensitivity when caring for transgender patients.
