Ken Fowler:

I visited my doctor earlier today because I’ve been feeling unwell for the past few days. I’ve been experiencing fatigue, nausea, and vomiting. After examining me and ordering some tests, my doctor mentioned something about a significant change in my creatinine levels and advised me to come to the emergency department. He instructed me to bring the test results with me. The test results from today showed a creatinine level of 3.2 mg/dL, whereas a month ago, it was 1.1 mg/dL, and there was also a urine protein reading of 400 mg microalbuminuria. To be honest, I don’t fully understand what all of this means.

Ken Fowler:

I’ve also been feeling extremely tired, probably due to the continuous vomiting and lack of food.

Healthcare Provider:

Can you describe the appearance of your vomit?

Ken Fowler:

It appears clear or sometimes contains residual food particles, although I haven’t eaten anything for over 24 hours.

Healthcare Provider:

Have you noticed any vomiting that resembles blood or coffee grounds?

Ken Fowler:

No, thankfully, I haven’t experienced anything like that.

Healthcare Provider:

Are there any pain or other symptoms accompanying your nausea and vomiting?

Ken Fowler:

I did strain my back last week while lifting a heavy box from my basement. The pain has been improving, but it’s unrelated to my current nausea and vomiting. Also, I’ve been feeling lightheaded when I stand up too quickly.

Healthcare Provider:

Could you provide some information about your current or past medical conditions?

Ken Fowler:

I have high blood pressure, and I’m on medication for it. Last month, I was informed that my high blood pressure had caused some kidney damage, with mentions of protein in my urine. About a week ago, I strained my back while moving a heavy box during a garage cleanup. It was quite painful, so I took something for the pain. My back has improved significantly since then, but that’s when the nausea and vomiting started.

Are you currently taking any prescription medications?

Ken Fowler:

I’m taking lisinopril, metoprolol, and hydrochlorothiazide for my blood pressure. However, I haven’t taken any of them in the last 24 hours because I’ve been feeling so sick and couldn’t keep the pills down.

Healthcare Provider:

Are you using any over-the-counter or herbal medications?

Ken Fowler:

At the moment, I’m not taking anything, but when I strained my back, I started using Naproxen. That was approximately 7-10 days ago, and I took a couple of pills twice a day. It really helped with the pain. Do you think that might have affected my stomach and caused the nausea?
